Dear Parent/Legal Guardian

RE: Grade 7 and 8 Hepatitis B, HPV and Meningococcal Clinics

Each school year grade 7 students have an opportunity to receive three vaccines at school offered by Public Health. These vaccines include Hepatitis B, Meningococcal ACYW-135,  Human Papillomavirus (HPV).

As result of the COVID-19 pandemic, the vaccines were not offered at school, but  appointments are now available at:

  • 99 Regina Street South, Waterloo
  • 99 Beechwood Road, Cambridge

If your child is in Grade 7 or 8 they are eligible to start their school program vaccines or  complete their series if they have already started them. Please note that Hepatitis B and HPV  vaccines require 2 doses (min 6 months apart) to fully protect against these diseases.  Meningococcal ACYW-135 is provided as a one-time dose. Meningococcal vaccine is required  as per the Immunization of School Pupils Act.

Next Steps:

  1. Before booking an appointment, ensure that all vaccinations received from your health care  provider or out of province have been reported to Public Health. There are several ways to  report your child’s immunization record:
  1. Use this link to book a school program vaccine appointment online https://bit.ly/3pyLr8f OR call 519-575-4400 ext. 5001. Routine school vaccines are also available through local  health care providers.

If you have previously declined these immunizations or have a legal exemption on  file at Public Health, please disregard this letter

If you have questions about the School Vaccine Program catch-up process, please call 519- 575-4400 ext. 5001.
